This is a sample of an IEP infused skill grid matrix that can be used to embed IEP goals throughout a typical school day for your child.

Data Collection Matrix

This is an example of how IEP goals can be tracked across a typical school day to indicate whether or not your child is meeting his/her IEP goals. There are different types that will show the level of support your child may need to reach IEP goals.
